When you refer to Comfort Care Homecare, you can expect a smooth intake process, timely start of care, and consistent communication back to your office. We make it easy—every time.

Submit Your Referrals

Please submit referrals by fax, email, or through your facility’s electronic referral platform. Our intake team monitors all channels consistently and processes referrals promptly.

What to Include

To process your referral quickly, please include:
 
• Patient name, date of birth, address, and contact information
• Insurance information (Medicare, managed care, or private pay)
• Primary diagnosis
• Physician orders for home health
• Most recent visit notes or discharge summary (if available)
• Preferred start of care date (if applicable)

What Happens After You Refer

Once we receive your referral, our intake team reviews it promptly and contacts the patient or family to coordinate the start of care. If there are any clinical questions or documentation needs, we will reach out to your office directly rather than letting things sit. Our goal is to make every referral feel effortless on your end—and to make sure your patient is in good hands from the moment you send it.

Service Area

We accept referrals for patients located in Palm Beach, Martin, St. Lucie, Okeechobee, and Indian River counties. If you are unsure whether a patient falls within our service area, call us and we will confirm immediately.

Insurances Accepted

We accept Traditional Medicare and select managed care plans. Private pay arrangements are also available. Contact our intake team with any questions about a specific plan.
